Official URL: http://www.euskomedia.org/analitica/10783 AbstractAssessment of patients who are candidates to be transferred to another sanitary or socio-sanitary care level by means of the use of an integral assessment tool that evaluates and classifies patients depending on their needs and levels of dependency with the objective of circulating them within the most adequate mechanism. This facilitates a stable flow of patients between the various levels of hospital care and between those levels and the socio-sanitary care mechanisms. An introduction is made of the results for the year 2004 in Gipuzkoa.
